Incident 5 May 1983 en route MIA-NAS when the No.2 engine failed, the crew elected to return to Miami, during which the No.1 & No.3 engines also failed. The aircraft made an unpowered descent to 4,000 feet, at which point the crew managed to restart engine No.2 and make a single engined emergency landing at Miami.
Later to Delta Air Lines N334EA & N788DL, Interlease Aviation N788DL, TradeWinds, Tropical International Airways & Interlease Aviation N826GR. Scrapped in April 2004 at Greenwood-Leflore.
