Manager Evaluation Form

This is a survey to be filled by the employees for evaluating the manager. The form contains 4 titles to be evaluated: administration, interpersonal, individual and leadership. Also an additional note box is available. PSW version of the template allows to save the forms by the name of the manager. Hence, collections of evaluation forms can be created for different managers online.
