Using suggestions recently posted by Michael Baird and Kevin Cole, I attached the included ear plug set to my iPhone4s so I could use it as a cable release for the iPhone camera. Then I attached the assembly to my monopod via a Diff Case. The Diff Case has mounting holes that accept a standard 1/4-20 screw to mount it on a tripod in either the horizontal or vertical direction. It has an optional second cover (shown here tilted back from the face of the iPhone) and a carabiner ring (not shown) which attaches to the lower left corner of the case. I bought both plus the case for $41.73. See www.diffcase.com.
For the cable release, you squeeze on the up-volume (+) end of the ear button volume switch (the rectangular thing you see near the upper left corner of the iPhone. I couldn't get it to work at first until I disconnected, then firmly reinserted the phone jack (it took some effeort to get it fully seated). Now it works great!
