HCM is a genetic cardiomyopathy caused by mutations of the cardiac sarcomere, resulting in left ventricle wall thickening as well as a diverse clinical course including sudden death, heart failure, and stroke. The prevalence of HCM in the general population is 1 in 500 adults.⠀

Most common symptoms include dyspnea on exertion, orthopnea, chest pain, palpitations, presyncope/syncope or edema.⠀

On physical examination may have a harsh crescendo-decrescendo systolic murmur.⠀

Decreased preload worsens the obstruction and murmur intensity (Valsalva, Squat to standing, dehydration & diuretics) & Increased preload decreases the obstruction and murmur intensity (Leg raising).⠀

Decreased afterload worsens the obstruction and murmur (ACE-I's) and vice versa.⠀
