B-52H-135-BW, s/n 60-0006 (MSN 464371). Last assigned 34th BS, 17th BW, Wright-Patterson AFB, OH. Written off 30 May 1974. Crashed on runway during GCA approach at Wright-Patterson AFB. Rudder and elevator failure caused loss of control. Fortunately all seven crewmembers survived.
