What Are the Recommended Treatments?

Acne is a condition of the glands that secrete sebum at the root of the hair or hair follicles. This causes clogging of the pores of the skin and the appearance of various lesions such as comedones (black dots and white dots) and pimples (papules, pustules and even nodules). Depending on the lesions, acne has several levels of severity, ranging from very mild to very severe. Depending on the severity level, the treatments will be different.

Acne: The Different Stages of Severity

The recommendations are based on a classification developed by the GEA (Global Acne Evaluation), which classifies the disease into 5 degrees of increasing severity:

Grade 1: Very mild acne with virtually no lesions, rare comedones and scattered papules.

Grade 2: Easy to identify mild acne with a few open or closed comedones and a few papulo-pustules reaching less than half of the face.

Grade 3: Medium acne with numerous papulo-pustules, comedones and possibly a nodule. Lesions reach more than half of the face.

Grade 4: Severe acne, with numerous papulo-pustules, open or closed comedones and rare nodules and a lesion of the entire face.

Treatment for grade 2 and 3 acne is also local but combines both drugs.

The treatment of grade 4 acne includes the above-mentioned local treatment and may additionally include an oral antibiotic (cyclin).

For the treatment of grade 5 acne, oral isotretinoin is recommended from the outset.

Adequate maintenance therapy in the event of a gradual response is continued until the lesions are completely removed.

For grade 1 to 4 acne, treatment can be changed if it fails after 3 months. This period may be shorter for grade 4 acne to avoid the risk of scarring. In this case, oral isotretinoin may be proposed. Recall that this drug is prescribed only by specialists, the renewal of the prescription can be done by General Practitioners.

Before prescribing this treatment, it is necessary to be vigilant and to identify a possible:

Depression: The adolescent is particularly vulnerable to depression and the risk is higher in those with acne. Experts recommend to screen for depression with the appropriate means in any adolescent consultant for an acne and to seek a specialist consultation if necessary. As is the case with infectious diseases, it is advisable to limit the use of antibiotics to treat acne to situations where they are really needed and this in order to limit the emergence of resistant strains.

CONTRACEPTIVE PILLS

Finally, if some birth control pills can have a positive effect on acne, a woman who does not need contraception cannot be prescribed a contraceptive. The choice of the type of contraception should be a shared decision between the gynecologist and the woman taking into account her preferences and various risks, including the increased risk of venous thromboembolic disease for 3rd or 4th generation pills.

Anti-acne Diane 35 and its generics (also having contraceptive properties) can only be considered as a last resort if acne persists despite a well-conducted dermatological treatment, in consultation with the patient and a gynecologist, and taking into account the characteristics of the woman, especially regarding thromboembolic risk.
