During mating the male grabs her muzzle/nose "and hangs on for dear life..".
This often results in wounds that are sometimes fatal.
Apparently male otters are horny little devils; they have been known to forcibly mount young harbor seals and rape them to death. They even continue to mount dead otters.... and we call them cute....
