AGrace Sub-Acute & Skilled Care was established on June 2009 after formally taking over the formerly known San Jose Sub-Acute. Since then, we have re-modeled as much as 80% of the building. We are continously trying to improve the overall appearance and actual care of the facility. AGrace Management Staff has a combined over 80 years of experience in the Health Care Industry.
