Tax Form 8941
Taxes are due, hope you're ready or have an extension in place...if you need a Credit for Small Employer, the 8941 form is the one for health insurance premiums.

The I&M Rail Link never enjoyed great health and could probably be regarded as a small employer. It sold out to DM&E a few years after this photo.
