I did this after i saw a fellow flickr user with a similar photo. low shutter speed, and used time delay. The B&W and the Noise adds a different surreal effect.

 

Used an ikea handmade glass lamp as the light source from the top right-hand corner. Might try some hockneyized or lomo self-portrait next time... Please check out my other works...
