reverse polarity plug

This plug top was wired incorrectly with reverse polarity.

Not particularly uncommon, however, it still had 4 tags attached from previous companies who had tested and passed it over the last few years. You don't need fancy test equipment to see that this lead fails, you just need to unplug it look.
