look carefully at the wheel... you think a train can't get a flat tire? It can't, it gets a flat wheel. Car was bad-ordered after developing potentially serious flat spots on several wheels. If left alone, flat spots can cause injury or damage to people and objects inside the car, but it also risks derailing as that spot is like taking a sledghammer to the car and rail at 90 MPH. A crane would later be brought in (when I wasn't there grr) to lift the car up for wheel replacement.
