Blackheads, White pimples, cysts… juvenile acne comes in different forms and colors, but still upsets the self-esteem of adolescents. Overview of behaviors and treatments to limit this skin disease.

 

Who has never feared the appearance of a pimple in the middle of the forehead on the morning of a first date? Or pounded against a colony of black dots at the corner of the nose? Juvenile acne affects 80% of adolescents. This disease is all the more difficult to live in the era of the reign of appearance. On social networks, smiling faces with perfect skin put teens in front of their pimples, while they are in the midst of building their identity.

 

“The first thing to remember is that juvenile acne is only a transient disease, emphasizes Dr. Olivier Gaide, head of the interventional dermatology unit of the University Hospital Centre vaudois (CHUV). There is absolutely no risk, except for scars. “A bad phase to pass. Which does not mean that there is nothing to be done to combat these teenage pimples. Depending on the type of acne, the solution to choose varies. So, it is necessary to know first of all what acne we are talking about.

 

At Each Acne Its Pimple

 

At puberty, the influx of hormones transforms the sebaceous glands. These glands are responsible for producing sebum, an oily substance that protects our skin from external aggressions. Acne results from their stimulation. They produce excess sebum, which clogs the skin pores. Then appear black and white dots, which are related to retentional acne. When the bacterium Propionibacterium acnes gets involved, the pimples ignite and blush. Sometimes these pimples turn into white pustules containing pus. This form is called papulo-pustular acne. When the situation worsens, it is called nodulo-cystic acne, which is characterized by small balls under the skin.

 

Acne does not affect everyone the same way. Genetics plays an important role. Look at the class photos of your parents: if their face was covered with pimples in adolescence, there is a good chance that yours will be too! As for the role of diet, some doctors blame milk and sugar, others are more moderate. This is the case of Dr. Gaide: “only draconian diets have an effect. Excessive consumption of protein supplements for example. As this is the equivalent of 60 liters of milk per day, no risk with the morning cereal bowl.

 

DIY – Do-It-Yourself?

 

First advice that is never repeated enough: leave your acne alone! Piercing its pimples leads to scars that cannot be easily removed. Some practices allow to minimize acne. Even if pimples do not reveal any bad hygiene, washing your face once a day helps to get rid of excess sebum. For hydration, avoid fashionable BB and CC creams, and turn to non-comedogenic products intended for oily skin. Same thing for makeup. At the end of the day, always remove makeup well, then rinse your face with water to remove the remnants of the product.

 

On the internet, grandmother’s recipes abound. Cover the pimples with toothpaste, dab the pustules with lemon juice, and even cover your face with a lotion of leek juice. “These tips have no proven efficacy,” said Dr. Anne-Marie Calza, a pediatric dermatologist at the Skinpulse centre and associate physician at the University Hospitals of Geneva (HUG). Toothpaste, for example, contains titanium and zinc, which have an absorbent effect. But it also contains menthol, which is very allergenic. A clay mask is a good idea, if you do not have dry skin. “Essential oils should be approached with caution. These are highly concentrated products, the side effects of which are often forgotten. A bad dosage, and we risk adding other worries to her acne.

 

Dermatological Care

 

When products without a prescription do not work, it’s time to consult a dermatologist. For those reluctant to medication, laser and light therapy act on light to medium acne, but are not reimbursed by insurance. Otherwise, different drug solutions exist: retinoids (derivatives of vitamin A) for retention acne, benzoyl peroxide, antiseptics or antibiotics for inflammatory acne. However, the development of bacterial resistance was observed with antibiotic creams.

 

For young women, the pill is sometimes offered, but with caution. “The pills that work against acne are those that pose a risk of thrombosis,” explains Dr. Calza. Their prescription is therefore done in collaboration with gynecologists, who screen patients to avoid any problems. “Finally, faced with severe acne that has resisted other treatments, the patient is offered isotretinoin, such as the famous Roaccutan.

 

Regardless of the treatment adopted to treat acne, one should not lose sight of its impact on the self-esteem of the teenager. Pimples can represent a real source of stress. Stress is an aggravating factor in acne. To break this vicious circle, Dr. Gaide advises to move to relaxation, hypnosis or even theater. Activities that allow the teenager to focus on something other than his buttons. Dedramatize and keep in mind the provisional nature of this disease: this is the best way to get through this complicated phase.

 

Roaccutan, Friend or Enemy?

 

Roaccutan has one big advantage: its active substance, isotretinoin, is considered the most effective treatment for severe acne. But it has several drawbacks. The first is not disputed by anyone: taken during pregnancy, it can cause serious fetal malformations. In women of childbearing age, therefore, it should be prescribed with immense caution, and only if safe contraception –or even two means of contraception-is used. Then he was accused of causing depression and even suicide in some people. Studies on this subject remain contradictory. But it is necessary to be attentive to mood changes in people who take it, and quickly consult in case of depression. Finally, Roaccutan is known to cause all kinds of annoying side effects. However, the situation is changing. “Despite the controversy, the drug is well tolerated,” says Dr. Gaide. Previously, there were marked side effects, such as severe drying of the skin and mucous membranes of the face, as well as headaches. Today, the prescribed doses are much lower. These side effects are no longer to be feared. “Still, this drug is not to be trivialized and is prescribed only as a last resort, when other treatments have failed.

 

In case of a change in mood, the specialist should be immediately warned. For women of childbearing age, only prescriptions for a month of medication are given. Safe contraception should be guaranteed, preferably with two methods of contraception.
